the easiest way to do this is to smith as much as you can to get him to do an Oop's and make something completely different from what you are trying to make.
same as the "Oops" Achievement from Record of Agarest War

Hey man I gave your solution a positive vote because there is nothing wrong with it. It looks like some a-hole gave all the solutions a bad vote for this game. Talk about a d-bag right lol.

This will randomly occur if you will try to create some items in the blacksmith guild, you will create a random item rather than the one you wanted, as achievement says it's happen a lot.

In the Blacksmith Guild when you are smiting an item there is a SMALL probability that the item you are trying to smith will fail and it will result in a completely useless weapon or accessory and you will lose the materials needed for that smiting job. When smiting if a full body picture of the blacksmith fills the screen it means your smiting failed for the trophy.

With all the smiting you will be doing going towards it would be wise to let this happen naturally instead of purposing trying to unlock it. Though when you are smiting high level weapons with rare materials it is advised that you save beforehand so you don't lose the materials if it does so happen to fail.

This achievement will come naturally for anyone who frequently creates items at the blacksmith. To unlock this, you must have the blacksmith fail creating an item. This is entirely luck-based but is bound to happen eventually. If you're intent on unlocking this, save your game and then craft everything possible, then reload after you get your achievement.

It comes out randomly and is extremely annoying.

When creating an item at a blacksmith, there is a chance that the blacksmith will get tired and forge something different from what you ordered.

Then the trophy will appear.
