You need to get roughly 2000 honor points ( or -2000) and 7500-10.000 Fame points
In order to get this achievement you need to have your Honor either maxed out or have it at the absolute bad boy bottom in addition to having the maximum amount of fame available.
Honor can be earned and lost and wearing a bandana prevents you from gaining or losing any honor and is typically something you should only wear if you are about to do something that is going to have a contradicting influence on your honor bar (example: trying to get a 5k bounty while you're being good). The bandana also prevents you from getting fame, so especially during the story related missions I recommend against wearing it.
You cannot lose fame. Fame gives you certain perks throughout the game, varrying from reduced bounties , legally stealing horses and other things.
You can obtain this achievement by playing through the game once. Completing all the story missions without the bandana will get you near the maximum amount of fame.
Several methods for fame in case you end up short:
Poker Cheating: This is the fastest , easiest and probably the way that is available whenever you wish to use it instead of looking for opportunities to get fame. You have to wear the elegant suit. When you are dealer, cheat by pressing Y. Push your arrow all the way to either the left or right and a guy will start pointing fingers. Now you're getting into a duel. Disarm him by shooting the right arm or the gun itself. When you do , the dead eye colors should immediatly disappear and you shoot the gun out of his hands. This will grant you +100 fame. Run back to the table and keep doing this untill no one is left, save&repeat or proceed to the next town.( you won't lose any honor for this btw). You will lose the money that you paid for playing at the poker table so if you are short on money you should primarily focus on the cheaper poker tables ( McFarlane's ranch, Armadillo)
Bounties:, especially the bounties alive give you +50 fame everytime you bring in an alive bounty.
Escapees : +50 fame is awarded if you catch escaping prisoners alive and return them to the wounded lawmen. That amount is less if you kill them ( +5)
Robbers: Leave the robbers or horsethieves alive for 50 fame, kill them and get 5.
Damsel in distress: The lady with the broken cart wheel and her beloved 5 companions hiding behind the cart: +50 if you only kill the men and keep the woman alive. ( if you dead eye all of the targets before they come out of their hiding places and kill them all immediatly the women will drop to her knees and beg you for mercy)
Bird Shooting or pelt collecting usually earns you about 20 fame.
Completing challenges will get you fame, depending on the level, it's usually around 5 to 100.
At Casa Madrugada, buy the property there (in case you die). Equip the elegant suit, which enables you to cheat at poker. In Casa Madrugada, there's a poker table for $50, so go to it and start a game of poker. As soon as you start dealing, press Y to cheat. If you don't balance the arrow at all, you'll fail and be caught cheating. This will lead to a duel. Winning the duel will get you +50 fame.
Go back inside the saloon to the poker table, rinse and repeat. I got +350 fame this way in just ten minutes.
Also, if you're aiming for the chilvary's dead side of this achievement, this little town is a great place for it, as you can kill all the citizens there, gain minus honor per kill, and no one will even try to report you for your crimes (some will fight back, but it's easier than fighting law enforcement).
EDIT: winning the duel by disarming your oponent rather than outright killing him with nets you +100 fame per duel... I've found that killing him via multiple shots to the face is less risky, though.

Note: While completing missions make sure that you do not have your bandana equipped if you own one. Wearing one while completing story missions or side missions will cause you to not be rewarded your fame or honor points from the mission.
Fame and Honor will come from progressing through the story missions. Every time you complete an objective you will be rewarded with Fame or Honor points. Raising and lowering your Honor level is a lot easier and will require less effort. Decide what path you want to take and stick to it.
This will unlock at some point later in the game. Complete as many activities as possible. Once you get close to the 100% completion this should unlock with any issues. Just make sure you don't stray your either good or bad honor, as it will just waste time. Raising your Fame bar will take longer than your honor bar so make sure you help people at the side of the road and complete all the misc tasks.
Fame builds up by doing missions, side missions, killing civilians, completing challenges, pretty much anything and it can never go down. Honor is affected on your actions and decisions throughout the game. Getting low Honor is a lot easier than getting it high so if you're having trouble building up your Honor, make a new save and start killing everyone on sight. Once your Fame's as high as it can be and your Honor is as low as it can get, the trophy will unlock.
