Talk to Joe in the armory at the Fire Station to start it. He will ask you to check on his brother at his house. When you get there head towards the basement and open the door but immediately run away as it is a trap. As long as you don't take damage the achievement will unlock.

To unlock this achievement, make your way into the basement of the Creelman place, and take a right at the bottom of the stairs. There, you’ll see a door. This is the cultist’s trap.
