Found a solution to tracker not going up for mods. The tracker for the achievement reads your progress based on how many mods you have unequipped. If you have several mods equipped in your weapons make sure to remove them before you make your next mod. Once you make a new mod your achievement tracker should match how many mods are sitting in your inventory. Friend just tested and he jumped from 53 percent to 73 percent on the 15 mod achievement.
Be sure to only make mods in your own world. Same goes for purchasing armor.

Got 8 mods so far. Achievement popped randomly. When i restarted the game. I think it is a little bugged.

The 5 mod one popped for me as I joined someone’s game. Weird..

Mods are acquired from defeating bosses and using the loot from them to 'craft' them back in Ward 13. As well as the boss specific loot you will also need to spend a small amount of Lumenite Crystals when crafting them. A list of the mods I personally found and the bosses the materials required drop from are as follows:
- Beckon - Crafted using the 'Sentinel Shard' acquired from the mini-boss 'Raze' on the world of 'Rhom'.
- Blink Token - Crafted using the 'Blink Spear Shard' acquired from the mini-boss 'Onslaught' on the world of 'Yaesha'.
- Breath of the Desert - Crafted using the 'Radioactive Skull' acquired from the mini-boss 'Scourge' on the world of 'Rhom'.
- Hot Shot - Purchased from McCabe in Ward 13 or else given by McCabe at the start if you choose 'Scrapper' as your archetype.
- Howler's Immunity - Crafted using the 'Hound Choker' acquired from the mini-boss 'Maul' on the world of 'Rhom'.
- Hunter's Mark - Purchased from McCabe in Ward 13 or else given by McCabe at the start if you choose 'Hunter' as your archetype.
- Iron Sentinel - Crafted using the 'Ancient Core' acquired from the mini-boss 'Ancient Construct' on the world of 'Rhom'.

Before buying received mods from a merchant (in the hub), make sure you make a save to the cloud/flash drive. It helped me to take out all the mods inserted into the weapon (I had 11 with me) into my backpack and then buy the remaining 4 pieces. Before this I tried to buy more without removing mods from the weapon, but it didn’t work!

As of April 2021, mods do not need to be removed from weapons. Mods that are installed on the weapon by default and cannot be removed are also taken into account. In total, there were 4 weapons with innate mods, 10 free and 1 inserted into the gun. The trophy was given immediately after receiving the 15th free mod.

I have 16 of them and the trophy did not fall.
