How to unlock the Madman's Paradise achievement in Remnant II - Definitive Guide
I will provide a bit of information to get you on the right track of what to look for.
World: N’Erud
Starting area: Forgotten Prison (reroll adventure if you didn’t start here.)
*Edit*: this is the world boss on this run.
Once you reach the area the eon vault - large open area with various other dungeon doors. It is now time to look for unnamed area that looks somewhat like a pyramid. Basically it is in a structure that is not part of another dungeon with orange lights. There will be an elevator inside and no other dungeon door on the map. Since it is procedurally generated I can’t provide much guidance other than look at all areas of the map and if you see a real door on the map it’s not it. Once you find it go up the elevator and the soul spark item will be inside (not much else in here to miss it). At this point you are ready to go back and present to Tal’Ratha. You have a choice that I won’t get into for spoiler reasons, but either option you choose will result in a battle. Depending on your choice there is a different reward. If you choose to fight it will give you an option to build a handgun called Nebula. If you choose the other way of concluding the mission you will get acidic jawbone (melee).

Where is this Tal’Ratha? Can you provide more info about him? How do I find him after collecting the item?

There are two potential bosses in N’Erud: the Guardian of N’Erud, Sha’Hala; and Tal’Ratha. If you don’t start at Seeker’s Rest, and you start in the Forgotten Prison, you’re on track to face off against Tal’Ratha at the end of this act.
Simply follow the quest and you’ll eventually end up in Tal’Ratha’s Refuge with the necessary quest item, and you’ll be given two options: be eaten, or not be eaten. The choice doesn’t matter here, you’ll fight off against Tal’Ratha either way. It does, however, change the boss’ form and the rewards. That’s about it though. We’d suggest being eaten, so you can also get the Was This Supposed To Happen? .
