How to unlock the Only Human achievement in Remnant II - Definitive Guide
The way I got this achievement was by killing the Doe and siding with the Ravager. The Doe won't fight back so there is no way to fail if you choose this option.
The Doe/ Ravager fight is the world boss for Yaesha. But there are multiple rolls (questlines) for worlds. I believe if you spawn in the Forgotten Grove you will have the Doe/Ravager quest. If you spawn somewhere else you will get a different quest and a different world boss. Reroll the world in adventure mode until you get the correct spawn (Forgotten Grove) for the Doe quest.
(Sorry for the vague first post so I hope this helps out as at the time of the original post I was not aware of the multiple quests (rolls) for worlds.)

Playing on the Steam Version, both Achievements popped when I killed the Doe and sided with the Ravager. Not sure if this is going to get patched, but it worked for me.

Only Human
Defeat a Boss in Single-Player Without Taking Any Damage
To unlock this achievement you currently have two different options. The first & easiest is to side with the Ravager and kill the Doe. The second option is to play on the lowest difficulty and run a pure DPS build to melt whatever boss you feel most comfortable killing without getting hit.
For method one you need to reroll the map Yaesha in adventure or campaign mode till you get the starting point the Forgotten Grove this will give you the Ravager storyline. Now simply play normally till you solve the Water Harp Puzzle you can then meet the Ravager. I have included a video with both solutions for the water harp netting you the Carnage in C-Minor achievement as well.
After talking to the Ravager you will have multiple options as to how to end this encounter. What you want to do is make sure you are ready then kill the Doe DO NOT attack the Ravager!! If done correctly you will get the Only Human achievement. This video explains exactly what to do.
I hope the guide was helpful & you will leave a like on the video. If you are going to down vote the guide then please tell me why so I can improve it.
Thanks & stay frosty!!!

I loaded up adventure mode for a new roll on Yaesha, the Forgotten Grove has to be the starting area for the ravager storyline (Forgotten Grove). I then set my game to Offline mode.
To turn on Offline mode. go to the system menu. From here, set the game to Offline mode.
When you get Yaesha, play through the world and if you’ve got the right roll you’ll reach the Forgotten Grove area.
After you solve the Water Harp Puzzle you can meet the Ravager. Once you do, talk to him and he’ll ask you a favour: to kill the doe sitting in front of him.
Grant his request and you’ll unlock the achievement

Quick note: the forgotten grove has to be the starting area for the ravager storyline. If your starting area is the red throne, you will have to reroll the world.

Easiest way is to side with Ravager and kill the Doe. The Doe will not fight back and you will not get damaged.
It will also grant the "Chaos" Achievement, even though we did not fight the Ravager.

While this might seem a daunting achievement from the off, fear not, being able to step back into various areas in Adventure mode, once you’ve completed a region, makes this all the more attainable.
In fact, there is even one boss fight that you can do that allows you to get this achievement without raising a finger. That’s right, folks; if you get to the Ravager fight in Yaesha (when you start in the Forbidden Grove), you can choose to kill the Doe before the fight starts and this will not only complete this region and World Boss, but it’ll unlock this achievement too. Sure, it feels cheap, but we’ll definitely sleep soundly in our beds tonight.

This method worked for me::
When meeting the Ravager in the Yaesha map and it offers you the choice to shoot the doe, do it, and the trophy will pop. I guess the game also considers it a boss.

If in the world of Yaesha (forest biome) at the very beginning of the first location you met an NPC ram covered with a root plague, then the boss at the end of this world is Wolf. Before the battle with him, the Wolf himself will offer you to kill the Hind. If you agree, there will be no fight with the boss, but it will be counted as passing the boss without damage and the trophy is yours. The Doe can also be saved, but then there will be a fight with the Wolf.

Easily obtained from the Abomination boss in the world of N'Erud. Used a fire whirlwind. He pulls him in and doesn’t let him take a step.
