How to unlock the The Web achievement in Remnant II - Definitive Guide

This will take place in the World "Losomn" in the sub region "Morrow Parish".
This will be during your time leading up to the nightweaver fight. (Can unlock after if you didn't do it at that time.)
You will first need to find 3 "Stone-Carved Dolls" located around Morrow Parrish. I believe they will be in different spots every playthrough because of the games randomness but I found one outside in the graveyard, one in a room on the second floor, and one in the attic.
Edit: You will also need the Prison Cell Key located on the third floor on a balcony outside the window. (Not shown in video) To open the door in the basement.
With all 3 dolls go to the basement and there will be a locked prison door with someone inside you can talk to. Make sure to go through the dialogue and give the dolls to the person in the cell. (While you are here make sure to listen to her song and write down the numbers in it. She sings the code to her safe on the second floor. My code was 2971 but could be different every playthrough. This chest is not related to the achievement though.) You can eventually ask her to unlock the door and inside will be a new Doll you can collect.
Take this new stone doll and travel to the tormented asylum by going through the web in the basement or fast traveling if you've already been there. Use this doll on the web in the prison cell in the basement to get the special weapon and achievement.

For the longest time I thought I had done something wrong or this was bugged, because no matter how long I talked to the doctor at the door, she would not give me the prompt to ask her to unlock the door as you mentioned when you say "when they open the door".
So, after a bit more exploring, I found a balcony where there is actually a key for that door. The balcony is up on the third floor, right next to the big hole in the floor.
I'm guessing you already HAD the key, and you were given the impression she was the one that was unlocking the door, but it's actually you (she's not actually there, after all) unlocking it with that Prison Cell Key.
So, after a bit more exploring, I found a balcony where there is actually a key for that door. The balcony is up on the third floor, right next to the big hole in the floor.
I'm guessing you already HAD the key, and you were given the impression she was the one that was unlocking the door, but it's actually you (she's not actually there, after all) unlocking it with that Prison Cell Key.

Sorry about the confusion, I edited the solution. I realized on my second playthrough you also need the key and the three dolls.

In the Morrow Sanitarium in Morrow Parish, which is part of the Nightweaver storyline in Losomn, you'll eventually end up in The Tormented Asylum. Downstairs in the dungeons, near where the locked up head sister was, you'll find a mysterious web. This is the Nightweaver's Web.
Now, there are various things you can put in the web to get an item, but the most straightforward is getting the Nightweaver Stone Doll from the Morrow Sanitorium and using that to put into the web.
Before you do that though, grab the 3 statues around the Sanitarium (because if you don’t, and don’t get the safe code for the office safe, you’ll miss the chance to get it the correct way).
The 3 statues can be found as follows:
Now, there are various things you can put in the web to get an item, but the most straightforward is getting the Nightweaver Stone Doll from the Morrow Sanitorium and using that to put into the web.
Before you do that though, grab the 3 statues around the Sanitarium (because if you don’t, and don’t get the safe code for the office safe, you’ll miss the chance to get it the correct way).
The 3 statues can be found as follows:
- The first one is in the courtyard out front, where you fight the Ripsaw Aberration.
- The second one is on the ground floor. If you come through the main double door entrance, go right and right again. It’s in that room.
- The third one is in the far corner of the attic room, the same floor that you go through the Elven rune door to Nimue’s Retreat - it’s literally the large room next to that door.
The key item to get the Nightweaver Stone Doll is the Prison Cell Key, which is on the front balcony, on the top floor. It’s in the attic room with a large hole in the centre of the floor. The window is open there that leads to said balcony.
Once you have all three statues and the key, give them to the sister locked in the cell (you get the passcode for the safe from her then), and finally, open the door with the key, and grab the Nightweaver Stone Doll.
All that needs to be done then, if you haven’t already, is to head to the web in the basement nearby and put the Soulkey Tribute you got from following the main path (for us, that was at the end of the Shattered Gallery) and you’ll be in The Tormented Asylum.
