How to unlock the Was This Supposed To Happen? achievement in Remnant II - Definitive Guide
An easy way to get this is to have Tal Ratha as your N'Erud World boss.
When you talk to him before the boss fight ask him about him wanting to eat you. He has a pretty serious kink, don't shame him.
And then agree to let him eat you, again, no shame if this is your thing.
After this very easy fight, perhaps easier than the normal fight against him. You will get the alternative drop. But this is not the reward.|
To get the reward you need to return to McCabe and have her craft the item. So make sure you have at least 1000 scrap, and some Lumenite on hand.
As soon as you craft the item, the achievement will pop.
Here is an excellent video from AbbyHour showing this. Giver her some likes if you use this method.

A few of the game’s World Bosses in Remnant 2 have what are known as ‘alternate rewards.’ Alternate rewards require you to perform a specific task in a boss fight in order to unlock it at the end.
For instance, the most obvious one is if you destroy the Nightweaver’s heart in both phases of the fight at the end of the Losomn world chapter. If you do, she’ll drop an alternate reward at the end - the Nightweaver’s Finger, which makes the Nightshade weapon.
The easiest one, however, is if you face off against Tal’Ratha in N’Erud. When you get to the fight you are offered two options: be eaten, or not be eaten. If you choose to be eaten you’ll unlock the Acidic Jawbone, which creates the Gas Giant.
Not only must you get the alternate reward, but you also need to visit McCabe and create said weapon using the alternate reward material. Do that and this is yours.
