When you reach level 4 with Alex, you unlock the skill variant for her Biohazard passive skill, which is the Epidemic mentioned in the achievement description. Select it on the customization screen. Once you start a match, all you need to do is spawn an infected zombie, and then spawn a few other zombies around them (the infection immediately passes to them). Zombies you don't spawn, but are already present in the world, can also count towards this.

I think maybe the requirements are lowered, I unlocked the skill at level 3

Yeah, I unlocked the skill yesterday at level three during a TA boosting session, but didn't get a chance to use it yet.

First unlock the skill variation you'll need for this. Then make sure you have a few Infection base items. What you need to do is use Infectious gun or Vapor to add Infectious to four enemies. I recommend using Infectious Vapor since it covers more ground as well as being cheaper.
