The No Damage Bonus is awarded in the Raid Mode for completing a level without being hit at all. Easiest done on the second level at the beach.
The pattern the enemies are spawning are really easy to remember. Listen to the sounds the enemies make and you will not be surprised. Always keep your distance and you will not have any problems with this.

The No Damage bonus is awarded to you when you complete a raid mission whilst, unsurprisingly, not receiving any damage. During the mission in the upper right corner of the HUD you will see an icon in the image of a person. This will be highlighted in white when you start the mission, but will grey out when you receive any form of hit or get damaged at all. This will need to still be highlighted by the end, in order to receive the bonus.
