Not a in depth guide to how to get this achievement but just something that I used to unlock it.
I’m no pro but I didn’t think the first 4 levels were that bad to get a S in, then after a couple of tries managed to get a S on village 2 and castle 2 then hit a real problem with factory and mad village 2. Personally I found these levels really difficult infact with factory 2 I was struggling at the first door with 3 archers, I started to think I’m never going to get through both the stages till the end until I stopped looked at the points being received, realised they the same as the earlier levels then after looking at the book in the starting room and seeing the s rank point requirement and realised from the easier 4 levels it wasn’t actually that high.
For factory 2 instead of worrying about the whole level I just concentrated on part 1 and used the magnum to kill anything tough and just practised and tested a way I was comfortable with and managed to get a 45 kill streak finishing with around 3 and a half minutes left and all the money I could collect and this got me the S requirement without even touching the 2nd level then when part 2 started just let the first enemy kill me and you still get the S rank.
For mad village again I used the magnum. I actually found getting a 41/41 kill streak on the first part actually really easy. Go right to the hut for the ten kills then go left and just stay in the tunnel and the enemies come to you round the house down to the graves once you get used to it really easy. With the 7 minutes I had left and all the money with the 41 kill streak you get around 450,000 points out of around the 650,000 you need. Then part 2 again isn’t to bad again I killed everything but messed up a few times and never really got a kill streak going but after this level I had the s rank so didn’t even need to do last 3 stages and just let a enemy kill me as soon as part 3 started.
Like I said not a guide as such just some info for people that are struggling with these two stages. As soon as I changed my tactic from trying to get from start to end and being careful with ammo to just using everything to blast through part of the stage I did both on the first try.

Yeah I agree. For Mad Village II it is best to go all out on the first two levels and get the S by the end. I ended up with over 750,000 from two levels which is much more needed to get the S rank.

With the Winters Expansion DLC this achievement gets incredibly easy.
Lady Dimitrescu is so stupid powerful it's unreal.
I hated this mode on release so I didn't play more than two stages.
Luckily Once you own the DLC, Chris is automatically unlocked. You don't need to play as Ethan at all, but you need:
All Stages A Ranked to unlock Karl Heisemberg
The Bloody River S Ranked to unlock Alcina Dimitrescu
If you haven't done so already, play as Chris to get your As and Ss until we have Dimitrescu
*As of writing I haven't played as Heisenberg at all
As Chris, what we want to do is sell the Dragoon and all the Rifle ammo for every stage.
Focus on upgrading your Handgun, and your fist. Also, in the Dukes Shop there is abilities to speed up your Outrage meter via gun or fist. Since we are using a handgun, we might as well grab that. Chris cannot heal himself but it's so easy to pick up healing abilities and just the fact his Outrage Ability is so stupid strong, we shouldn't really be losing any health.
When outraged your punches are super strong and you can fire your gun ridiculously fast. Makes obtaining at least an A a Cake Walk. You're going to do this for every stage and try to obtain as many abilities as you can. Since the ability pick ups are RNG, just grab what you feel is best for the area you are in.
I didn't look up any guides so sometimes I'd miss an enemies and not get the All Clear bonus. I honestly didn't care, this mode can suck it. I just ran through and punched everyone in the face. It's pretty easy to stun someone with your fist and then unload a clip into the red heart in their chest and make short work of the stronger enemies.
That stupid propeller dude? Just let him run into a wall and the punch the hell out of his back. Dude goes down like a joke.
Once we have Alcina Dimitrescu playable, we need to mop up any Ss we didn't get as Chris or Ethan.
For all the Normal Missions, I just leveled up my claws and high as they would go first and then worried about flies. If you can afford to, the cooldown her chair is a lifesaver if you're trying to preserve combos. I'd either S or SS levels in my first attempt.
Before walking out of the Dukes room, apply your lipsticks to boost your thrill meter so you can start rounds with thrill.
I only died once as Alcina Dimitrescu on Factory II as RNG was brutal and wasn't giving me any Healing Abilities. Dimitrescu isn't like Chris, she has her own healing item, and can heal herself, but it's easier to go all out and slay when you heal on kill. I always made sure to buy a couple extra healing items just in case things went south.
Other than that, SSs and SSSs came so simple as Lady Dimitrescu it was freaking stupid how strong she can be. Summoning her daughters to kill for you while you walk away is a god send.
Use her flies for enemies that normally would need a gun to reach.
Good Luck to anyone who still needs this Achievement. I had so many doubts this update would let me earn this.

How do you select Chris? I have the DLC but don’t have an option to select characters only select the Village level and then it starts?

If you've been having trouble with this like I was, good news. With the DLC, this is now trivial. Just finish (no rank requirements) the two new maps and you will unlock Heisenberg and Lady Dimitrescu. Just pick Lady D and upgrade her claws and the flies, then smash your way through the levels. Knowing the main routes helps, but it's not really that hard. Just pick health. melee and speed buffs. I got SSS on Mad Village 2 in one try.
The Mercenaries mode is available for purchase on the Extra Content Shop after you beat the game for the first time. While it may seem like an extremely difficult trophy to get, truth is that, for the most part, you'll only need to remember the spots where enemies spawn and follow the most efficient route, this becomes easier every time you replay an stage. There are random abilities that can be obtained by hitting blue orbs—during normal stages, I highly recommend taking the ones that increase damage, and during the harder ones choosing a combination of abilities that heal you by killing enemies and abilities that increase the damage of your guns. I'm absolutely horrible at this mode and was able to SSS all levels with Chris. My strategy requires a little RNG luck but once you get the beginning loadout you can finish it with a breeze. For the normal levels I sold everything and bought dragoon. I upgraded the power all the way and then w/e left into the remaining dragoon stats. Now the trick is to find a skill orb and roll till you get the ability that reloads your gun when you get a punch kill. When you get this start killing, dragoon is so over powered and once you max his right punch you have unlimited ammo. This strategy works on all normal levels and hard ones. I was able to get SSS on all levels with this and I'm the worst at this mode.
