After collecting all 4 Death Masks make your way to the graveyard and head into the Tomb. Place the 4 Death Masks (each one placed will release a chain that is securing the coffin) on their respected effigy’s.
After placing the 4th Death Mask the coffin will fall showing the Crimson Head prototype 1 laying in the coffin. Another cut seen will show you being sealed in the room by bars that now cover the exit.
The Crimson Head will then attack. Defeat him however you choose and upon his death the Trophy will pop and the bars will retract from your exit.

This is a Jill only storyline trophy. To earn this trophy you must defeat the Crimson Head Prototype boss when playing as Jill. The fight happens in an outer area of the mansion and requires the four Death Masks to initiate. You’ll earn this trophy once the boss is dead.

You simply need to defeat the Crimson Elder for this achievement to unlock. He's strong, about as fast as regular Crimsons and takes a punch but as Jill, you only need to fire three explosive rounds at him (Normal+) to defeat the monster. If you don't have it, use the Shotgun until he falls for good or the handgun. As far as I'm aware, decapitations are not possible.
