To get this achievement, you need to finish the game having saved both Rebecca and Jill.
To have Rebecca with you during the final boss in the lab, be sure to save her when she is about to be attacked by a Hunter upstairs in the Mansion (there will be a cutscene showing her in danger). Be sure to head there immediately to kill the Hunter, else she will die.
To get Jill, , you will need to collect 3 MO disks. One is in the Mansion, in the Tiger statue room, the other 2 are in the labs. Once you have them, head to the door closed by 3 locks in the labs, put them in and free Jill from the prison.
Then just finish the game as normal and this should pop during the final cutscene.

Take note you can't let the last boss kill Rebecca. The achievement won't unlock. Lol.

I've gone back and completed it multiple times but this won't unlock for me. Rebecca definitely survives the fight and I can see both her and Jill alive in the ending cutscene but this one isn't popping for me.

Edit: If anyone else has a similar problem, I waited a day and played through from my last save again and it unlocked for me. Restarting your xbox would probably be enough, just make sure not to save over your final save when you make your all clear save.

Make sure you save before the boss fight. it didn't pop the first time beating the boss. and to reload my save before the boss fight and it unlock after I killed him again. Seems like it happens a lot to the ending achievements in this game.

Thanks Chuun, that happened to me too - the last boss picked up Rebecca a few times and must have killed her the second time. It was my knife-only run so I was busy running a victory lap and didn't notice she wasn't with me in the helicopter. Reloaded save, tried again, success.

I can confirm you cannot let Rebecca die during the final boss fight. This will void the achievement.
In my run Rebecca got trapped in the corner and the Tyrant just kept swinging at her eventually killing her.

(Missable) This is a Chris only trophy and isn’t too hard to earn. Rebecca can take care of herself for most of the game; but you must be certain to save her from the Hunter (see That Was Nice of You) and check on her after defeating the Tyrant the first time. The final problem with Rebecca is she’ll join you during the final Tyrant fight, you must keep the Tyrant distracted enough so he doesn’t kill her. Saving Jill isn’t a problem as she’s in a cell until the very end of the game (see Sorry I Made You Wait). So simply keep Rebecca alive and remember to rescue Jill from the cell and you’ll earn this trophy after defeating the Tyrant for the second time.
