At the end of Chapter 4, choose to keep your device. When you reach the point where you can cross an ice covered beam or climb down an alternate path, choose the alternate path and climb down.
Eventuay you'll reach a ladder with a panel you can override. Choose to do so, and flip the switches as follows:
down, up, up, down, down, up, down
The panel will be overridden and your achievement will unlock.

Actually you get to this ladder whether you cross the beam or take the alternate path, so this achievement is obtainable whichever route you take.

This is on the route for doing the Grace Ending, after you have jumped out of the funicular air hatch. There are two different routes you can take (across the beam or along the path) - both of these routes have achievements so I made a manual save to prevent having to do another full playthrough to get all the achievements.
To get this achievement, chose to use the Ladder Override when you reach the option to use override or take the path decision (this is after you have either went over the beam or chosen the first path). You need to set display to 50, binary code for 50 is achieved by flicking the switches (from left to right) to:
OFF, ON, ON, OFF, OFF, ON, OFF
This is shown from 1:21:50 onwards in this video.
