During Chapter 3, you'll be exploring a huge apartment block spread over three floors. Make sure to enter ever single apartment - you'll know if you've been in one already because you'll have unlocked the door panel with the code which your AI gives you, and there's one with a broken lock which you can just enter.

Make sure you've examined all things in all rooms on the current floor before climbing up to the next level, as you can't go back down once you do! Also, do not take the beam shortcut when given the opportu6to do so, as you'll miss a couple of rooms.

The Dark Room, where Control turns the lights off, also counts for this - my achievement unlocked after examining everything in that room.

Adie will announce "I think this is the last room" when you've searched them all.

Gained by simply entering all 11 apartments in the Apartments area (starts just after you cross the first beam), you do not need to examine everything in the rooms to get this - you do however need to examine all the wish canisters you find in any of the rooms that have them though for the Wish Canisters achievements. Do this achievement while going for the Grace Ending.
