This achievement is missable and can only be done in Part 2. When tasked with reporting messes on the ship, you need to find all 8 possible messes. To do this, save your game before finding any, find five of them, and then reload the game to find the other three. Here is the list of the possible messes:
- Gullet stuck down the back of in the ship underwater
- The spilled potion on the deck
- The entire galley (go in kitchen and select the ingredients)

Encountered a bug with this on my first playthrough - did all eight messes as per the guide above and I'm stuck on 87% progress on console. I looked on the PC version and the achievement is showing as unlocked in-game on there, but nothing on my actual profile. Repeated the steps a few times on console and PC, but no luck.
Going to finish the rest of the game and see if I can come back and get on another playthrough, but if anyone has encountered this and fixed it, any tips would be much appreciated.

There are 8 places in total, and only 5 places can be recorded at a time, so files need to be read (save the task after obtaining it, record five places, read the file, and then record the other three).
1. Rats outside the captain's room.
2. The kitchen smells pungent.
3. Oiled windows in cargo hold.
4. Oiled floor of cargo hold.

So basically, What i did was record the first one.. Made A Save here.
Done the other 4, reloaded the original save with just one on there, then go and do the other 3 and achievement unlocks!
