This achievement requires a bit of dedication, since carrots are a particularly great healing item.
I would recommend focusing on this achievement as early as possible. When the carrot becomes available in the item shop (giant squid), by one as soon as you can. Be sure to use the carrot first when needing to heal. When returning to the town after each stage, I recommend buying a carrot, use it, buy another, etc. until you are completely healed and have one in your inventory.
Buying healing items can become expensive, so be sure to replay the first stage over and over if needed to earn some extra coin.
