This is actually very easy to do, as long as you focus on this achievement from the beginning (i.e don't utilize the hospital when exploring your first time). Otherwise, this will require and additional playthrough.
Just avoid talking to the bear at the hospital throughout the game, and it won't be an issue.
Instead of relying on the hospital to heal you, you can simply replay early levels to earn more money, which can be used to always keep your supply of healing items stocked. Also, keep in mind that you can always restart a stage at any point and you will begin with the amount of health you originally started with. This is helpful if you take a decent amount of damage early on. NOTE: you DO NOT retain any health items you used if you restart the stage, so plan accordingly.
