This is obtained in the path Burial Grounds. There will be several cards with pictures of graves on them.
Digging up a grave causes one of 3 effects.
- Poisons you
- Causes Curse Damage
- Gives you a random number of souls
If you're low on health click the gravestones until it kills you for the acheivement.
If you happen to have the offhand item Shovel equipped, the first grave will grant the item Bone Pearls and unlock this one as well.
