For this you have to equip an item into every available slot.
There are 15 slots. Your torch slot is filled by default by your candle so you need to find and equip each type of the other 14 in a single run. They must all be equipped at the same time.
The easiest way to unlock this is to get the (Blue) pants item Hermits Pockets. It automatically fills all empty inventory slots with junk on equip.
If you are just keeping track during your runs the slots are.
The slots are -
Torches
Helmets
Armor
Gauntlets
Pants
Boots
Offhand
Weapon
Mask
Jar
Necklace
Stones
Gems
Books
Scrolls

EDIT: this popped for me today with hermits pockets. Thanks for the reply below though - seems spotty to pop. Thumbs up as it’s a pain to fill the inventory normally.
I got the hermits pockets, all the junk filled the empty spots, but cheevo did not unlock. Wonder if this is glitchy or did you unlock it with the hermits pockets?
