Keep in mind when you play Double Candle mode the item you choose to start with is NOT the first item equiped. The first item you pick up DURING the game is the one that counts as first item and which you should carry with you the whole run.
Good items in brown chests to keep during the whole run are the following:
- Wading Rags (Intimidate poisonous creatures om attack)
- Soul Stealer (+3 Souls on kill)
- Catalyst (Doubles gain from next 3 Stat Boosts)
- Creepy Doll (Dungeons have +2 cards)
- Jewel (+4 speed, - 1 heart)
- Poison (book, +4 Poison to target)
- Timeless (book, wait a turn)
- Exit Phase (book, Teleport to Exit)
- Cure (book, Cure Poison and Heal 3)

I just thought I'd mention this here. So I just went for the first mask item I got, and held onto it the entire game. Not that it matters, but it was the Weightless mask. I finished my run, and waited for the achievement to pop, and it didn't. I reviewed my final score screen, and saw that the mask was still there. However, I noticed that there was a (2) next to it. I also noticed another (2) next to the clarity gem which I had remembered seeing the Viscid Union, the green slimer man that gobbles your items until you kill him, had eaten that clarity gem. So I'm guessing in the final room I didn't notice that Green Gobble man ate my mask at one point because I had the Pole Arm weapon equipped. I know I had the mask when I won, but it seems that if he gobbles your starting item, you won't unlock the achievement.
