The rock is a scroll item wich deals 3 damage when used, if it kills the target, you get the rock back.
There's two reliable ways (that I know of) to kill all enemies in a dungeon with it.
1.Low damage+Hermit's Tiara-The idea here is lowering the health of each enemy 1 hp at a time, thanks to the tiara's effect the enemies won't retaliate as long as you have enough speed, and once the enemies are at 3hp or less, use the rock.
2.Graves room with only Roaming Poisonbeads-Sometimes, grave rooms will only have 1 or 2 poisonbeads, these enemies have only 3hp on normal difficulty, so they can be oneshoted with the rock, just make sure you have enough health to survive the poison.
3.Start with the rock using Double Candle-Pointed out by the comments down below, if you have the Double Candle unlocked, start with the rock and use it to kill the enemies of the first room, hit the enemies first to lower their health below 3hp first.

The quickest way to unlock this achievement is to start the game with the Double Candle and the Stone as a started item.
You will have enough stats to kill all the ennemies with the stone in the first floor.
