Easily done if you start a new game with the double candle.
Pick any spoon as your starting item.
Then you just need to survive long enough without discarding/swapping the spoon that you can reach a Reprieve exit.
Enter the reprieve level with the owl logo and attack him. You'll likely die, but it still counts for the achievement.

It is important to note that you have to actually hit the owl, which means you either have to outspeed it or be able to tank a hit from it. If it oneshots you on turn 1 before your attack the achievement will not unlock.

I got this while holding the spoon and using the scroll that does 15 damage on the owl. Didn't have to actually attack the owl so my negative defense didn't matter.
