How to unlock the Poor Choices achievement in Ring of Pain - Definitive Guide
For this achievement, you must equip an item that offers negative health attributes that will cause you to die. Unfortunately, for this achievement, the stats are removed from your total health, not your current health. So, to make this possible, you will need to get your max health super low.
To do this, you will be looking out for a room called 'Fountain of Life'. But before you enter the room, earn as many souls as you can and don't try spending any just yet. Once you have all the souls you can, enter the Fountain of Life.
Each time you activate the fountain in this room, you gain HP, but lose 1 max HP. Keep activating the Fountain until you reach 1 HP, but careful not to activate it too many times or else you will die. If you haven't already, you will unlock an another achievement:
Anyway, hopefully you are well equipped enough that you don't die at the first enemy you meet. The goal is to get to the nearest treasure chest. Open it up and keep re-rolling until you find an item that offers at least -1 Health. As soon as you make the purchase, you will die and get the achievement.
IMPORTANT NOTE:
I HIGHLY recommend saving this achievement for last (or close to last). The reason being, unlocking this achievement unlocks a new scroll called Transcendence. Transcendence will destroy all of your equipment but lets you keep all the stats from the equipment. This can be a useful scroll if you are fully equipped early on with high stat gear and lots of souls to resupply your items. However, it mostly appears in the last couple of levels, where you are probably using gear for their useful abilities and not necessarily their stats. But the biggest issue is that it seems to spawn as the stage scroll for the last couple of stages a very high percentage of the time. The scroll that used to often appear in these rooms was 'Last Form', the best scroll in my opinion and a life saver if it appears before the boss fight. Instead, I find Transcendence in the room before the boss like, 90% of the time.

I personally recommend unlocking this achievement only once you already can play the inverted candle mode, where you will start the game with only one health point. In the first dungeon just equip any object that takes away a health point and you will lose, unlocking the achievement.
