How to unlock the A Hard Carry achievement in Ring of Pain - Definitive Guide
This is probably one of the hardest achievement in the game after "complete a run without any epic or legendary item", but with a bit of trying it can be done without too many problems.
The spoon is a challenge grey item that goes in the weapons slot, and it has very bad stats. After winning a run with the spoon, you will unlock the bronze spoon, than the silver, the golden, and in the end the diamond spoon, so to unlock this achievement you have to complete a total of 4 runs.
Spoon -2atk -2def +3health
Bronze Spoon -3atk -3def +5health
Silver Spoon -4atk -4def +7health
Golden Spoon -5atk -5def +9health
Diamond Spoon -6atk -6def +11health
So you DON'T have to win a run with the diamond spoon to unlock this achievement, but with the spoon fully upgraded you will unlock the inverted candle mode too, where you will have to complete a run to win another achievement.
I personally recommend playing on normal difficult in double candle mode, so you don't have to look for the spoon every time. Feel free to restart if you don't find good enough items in the first dungeon.

Although I struggled at first with this, in the end I beat the game on light ending using Hermit's tiara for freeze, and near end picked up the backpack which allows you to pick up as many items as you like, and it only counts as 3. Of course this is luck based and you might get it first try, or it might take you 100 attempts.........but this is how I finished.

I thought getting through these runs would suck since the spoon takes the weapon slot, preventing the usual 'Hermits Tiara' + 'Polearm' run.
But then I discovered how absolutely broken 'Soul Scream' and 'Stone Scream' were.
So start a run with double candle and take the tiara, then keep restarting until you get the spoon on floor 1. Then just play as normal going for mainly speed until you get blue rarity items appearing and hope you get one.
By the end of most runs, these spells were dealing nearly 200 damage, making the final dark boss and absolute cake walk.
But then I discovered how absolutely broken 'Soul Scream' and 'Stone Scream' were.
