This is the Owl creature in the Reprieve room which has an owl image above the exit.
In order to get this achievement you must be able to attack him at least once and survive a hit from him,
He's pretty strong so it may be better to do it later in a run.

You can use one of those +20 defense on next turn, no damage next turn, freeze, or entangle him.

Another easy-ish way is to unlock and draw the quirky ‘Chaotic Underwear’ on an early stage. This gives you +10 speed but also reverses combat attack order - lowest speed attacks first. Owl has 17 speed so if you find the item early then you can stay under that and try to find a Reprieve stage. These tend to appear early too so it’s worth bearing in mind.

Something that is not obvious based on the description is that you need to land damage on the Owl for it to register. You can do this in a number of ways, like waiting until you get enough speed so that you hit first or building up enough armor that you can resist a hit, but these all take some patience, and you won't always get the Reprieve door in the later depths (nor would you want to potentially waste an near entire run for that).
The way I did this is using the Chaotic Underwear, which is a grey helmet that reverses the speed order, so if you're faster, you attack second, and if you're slower, you attack first. This will let you get the first hit in very early on. You'll definitely die, but you'll get the achievement too!
