Ring of Pain

After completing a run that you chose to light the candle you will unlock the Double Candle which lets you select a common item to start your run with.

Choose the Misting Vessel which grants 10% Stealth for every empty inventory slot and starts you with 130% stealth chance (counter stops at 100% but this allows for picking up 3 more items). This also has a chance of spawning for you in room 1 without having to first beat the game.

Proceed to stealth past everything straight to the exit, don't pick doors with events behind them like crossroads, volatile etc.

Exploding enemies killing themselves and other creatures won't count aslong as you don't personally choose to attack something.
