Complete the Dark Ending on Medium difficulty to unlock Time Rift

After Community Content Update you have to reach depth 12 on Medium to unlock Time Rift.

Start a game on Normal with Double Candle using Time Rift, try to prioritize armor to parry and some HP, find a creature that deal poison damage and try to sneak him until you reach 20 poison on yourself.

I've completed the dark ending on medium for candle and double candle but haven't unlocked Time Rift. Which game mode is this for?

The item unlocked for me as a reward for completing the dark ending on Medium with Double Candle if I remember correctly… maybe there is another criteria you need to meet in order to unlock it but I don’t think you need to do something special. If someone unlocked the item by other means leave it on the comments so I can update the guide accordingly. Edit: Just checked my ingame stadistics and I was correct, you get Time Rift as a reward for completing Dark Ending on Medium. The achievement is called "Challenge Increases" by the way.

I've got that achievement, but no Time Rift.

Go to collection > Achievements (there you can see the reward items tied to achievements), then search for "Challenge Increases" and see if you have the reward unlocked. It should look like this: https://bit.ly/3EPb6Aj
