This solution contains moderate spoilers to the weapons you can obtain.
At the end of act 4 you will obtain a special weapon called Azoth which is a glove that fires energy (this is a story item so it's a certain that you will obtain it with progression) You will get in game instructions on how to fire it.
There are so many places that this can be achieved when you have the glove but this is a solution so one really does need to be listed;
Straight after you obtain the glove and walk back out into the hall where you 'struck a pose' enemies will drop from the roof, charge the glove and as they are coming towards you they should bunch up, If you didn't get it on the first group as you walk further down the hall keep it charged and as soon as the second group drops fire it, the second place is pretty consistent as they always drop in the same place.
Note: As mentioned there are many places to get this so don't worry if missed in the places mentioned.

Killing three enemies or more with one attack is tricky do with regular melee weapons, as it is hard to hit every enemy in a group at the same time. Rather, get a Golden Test Tube and toss it at a group of three enemies. If they all die, the achievement will unlock.

I would like another confirmation to be sure, but it would appear that you can get this with the "Meat-Mincer" in Act IV as long as you kill 3 creatures within a single arm-extension. I ran through them, killing them in a row but definitely NOT at once. So if true, you can simply extend your arms and walk through a crowd. (That being said, use of Azoth, as stated above, is still the BEST tactic.)
