This achievement is called "The Survivors" and is awarded for completing Act 2 of the game which is the primary introduction to the story of the zombies and the first real fight scenes. Simply follow the games directions and this unlocks after Spoiler Spoiler Spoiler.

At least one hot chick ends up dead... very sad. Also I think this unlocked when I was attacked by the mysterious villian with the gold helmet, then I woke up and saw a definitively crazy man and a hot zombie nurse.
