This is a scripted event that occurs towards the end of Chapter 4 of the game (4th teen play through).
You will meet Zoe by the side of Road 96 and progress your way together, the usual way, until the end of the chapter.
Right at the end, you will be presented with two choices. Regardless of which you pick, the achievement will unlock upon making the choice.

This trophy will pop when you escape the border with Zoe. After playing several chapters that involve Zoe, you will reach the end of her storyline when you meet her in the chapter "Road 96" (it will probably be your third/fourth time playing this chapter). Just keep playing and make an important choice at the end of the episode, and the trophy will be yours.

The earliest you can get this is episode 4, as i believe you need to have three teens escape the border and have Zoe's percentage high enough.
You will meet her at ROAD 96, and continue as normal to the border crossing and head off with her. At some point near the escape you will be given the choice to save her or save yourself.
Either option will give you the achievement.
