After Eustace dies his bell will be in the graveyard in Peak District. It will be on the side of the building where he was standing giving you a call out of when enemies are approaching. The quest is "Grave Robbers" where he is standing in this position. Pick up the bell and you will get the achievement. The location directly below the "An Interesting location" icon.
