This achievement is tied directly to the following:
Should you fail to obtain the 100% reputation rating, you will be unable to get this achievement.
Upon achieving the 100% reputation, you will be awarded the Key of Outlaw. Take this key to the Nine Ladies Stone Circle, Found North of Merlin, and open the second door from the left. Picking up the Outlaw's Backpack from the altar will unlock this achievement.

what makes you fail to obtain 100%? do you have to basically succeed every encounter, and refuse gold everytime? also, do you get more reputation if you clear a camp a second time after enemies come again?

There are a number of ways to fail the 100%:
If you fail a quest or to save someone being executed, you will fail.
If you sell stolen goods to a merchant and don't turn them in to a Steward, you will fail.
If you accept money from someone being executed instead of telling them to keep it, you will fail.
There is only just enough reputation sources to get 100%. Completing a camp again will not award you with any more reputation.
