This Is obtainable In Mission 2 The Search For Soot
After Finding the Button For The Secret Area (Which Will Unlock No Stone Unturned Achievement) Simply Turn Around And Walk Back On Yourself To See The Area
On The Right Hand Side There's An open Case With A Floppy Disc Inside Pick The Disc Up And the Achievement Will Unlock

There is also one in the Chop Shop found whilst doing one of the Downtown side missions to find the Mayor's nieces missing SUX.
Can be found in the side office next to where the SUX is located.

OCP skill disks are a sort-of collectible that, when found, immediately give you a skill point. They're rare, and found typically tucked away near side objectives. The first one I found was in the Downtown area during the second main mission, "In Search of Soot," during the side mission "Stolen Vehicle." Inside the chop shop that this side mission takes you to, there's a room right by the garage door with an OCP skill disk. Clip below.

I found easily another five or so during my time with this game, so you'll likely find more, but the Steam guide linked in the roadmap also notes locations of these skill disks.
