How to unlock the The Perfect Drug achievement in Rock Band 3 - Definitive Guide
For this achievement you need to hit 200 consecutive notes without missing any or landing extra hits. With drums every hit counts as a note, so if you have to hit two pads and a bass pedal all at once that actually counts as three notes. This achievement can be unlocked on any difficulty (even easy!), but only on drums. This song was a community favorite for quite some time, so I don't think you'll be disappointed.

Instead of playing the song in Quickplay or any other mode you should find the goal at Main Menu -> Career -> My Goals -> Drums -> Downloaded Song Goals -> The Perfect Drug. Playing the song from here gives you a counter in the top right corner of the screen that tells you how long your current streak is. It will glow green once you've reached your goal of 200 notes.
If you break your combo before hitting 200 notes I recommend starting the song over. It only gets more complicated as it goes on. You could potentially have some luck with the mellow outro, but it's not all that long so you're likely to run out of notes to hit before you reach a streak of 200.

Achievements in Rock Band 3 are all tied to Career Goals in the game. This one is tied to a goal for drums, so it can only be earned on drums. The same goes for Face Melter and guitar, Mercurial Vocalist and vocals, HOPO-cidal, etc.

Okay, first thing's first. The achievement simply states that you must hit 200 notes in a row on the song. It does not specify that it requires an instrument, which can lead to many failed attempts at it and much rage. To specify, this MUST BE DONE ON DRUMS. If you have the song and head into the My Goals section under career, and from there go into Drums -> Downloaded Songs, the little challenge which when completed unlocked the achievement will be there.
So, for the tl;drers, 200 note streak ON DRUMS, WHETHER PRO OR STANDARD. Nothing else will work.
Luckily, this can be done on any difficulty, as a 200 note streak on expert on a song of this caliber is quite a hard feat to accomplish. I personally achieved it on hard, but play whatever you feel comfortable with.
On drums, every single individual note counts as one towards your streak, so, for example, the opening 3 hits of the song on expert (BK-GK-R) would count as a 5-note combo rather than a 3 note combo. This has also been the case on every previously released Rock Band title, however is not true for Guitar Hero titles. Obviously there's a major helping hand with that being there; the achievement would be a fair deal harder without it.
I personally suggest you initiate the song using the My Goals menu mentioned before, as under the score will be a little counter when playing through this. This keeps track of the highest note streak you've attained within the run, which makes it far easier to figure out whether you've obtained it or not.
As an added bonus, completing this song will also give you progress on the Dave Grohl Band achievement, so it's win win.

It's worth mentioning that the achievement doesn't pop until you successfully finish the song.
Don't do what I did and keep restarting the song on Expert until you get the 200-note streak at the start... then fail the breakbeat section in the middle.
On Expert or Hard, there are more than enough notes in the outro ("without you everything falls apart") to get the streak easily there.

You can turn no-fail mode on this achievement; you'd then only need to get 200 consecutive notes at some point without worrying about being good enough to pass the song overall (so you don't have to worry about passing the hardest parts of the track; this is a pretty difficult song on drums but the beginning and end are the easiest parts).

For those who choose to play on expert drums. Best bet is to LRRLRRL for the yellow red red yellow red red. This seemed to work easiest for me because it allows for the bass kicks to be hit on Rights. Not too tough on expert seeing as you can hit 200 notes pretty quickly if you can maintain the streak. Fun song to play.
This must be done on Drums and can be done on any difficulty. It is a challenging song but there is a long outro that is the same pattern repeated for quite some time. Play on a difficulty you are comfortable on and you should be able to manage this in the outro on any difficulty if you are struggling. This song also counts toward Dave Grohl Band.
