If you're having trouble getting this, here are some tips:

It may sound cheap, but PAUSE and get ready for every single measure. Pausing the game brings up a menu which only covers up a small portion of the left side of the screen leaving the measures in clear view for you to look at 90% of the time. Time to stop and think is a great commodity in this game.

Also, use your SHIELD. If you get to a part where you don't think the pause trick will get it done, tilt your guitar to hit the shield. This will keep enemies from hitting you which will break your combo, you just have to make sure you don't make any mistakes in this time, as that WILL break the combo.

Great tips! I would also suggest going for this on the first Bonus level as there is a steady flow of enemies and it's not such a big deal to start over.

^ well it doesnt matter cause to finish the game some levels require a 100x multiplier for the medals

^ it does matter if you'd rather not keep playing this crappy game just to get all the medals and achievements and just want to get what you can and be done with it. In that case, going for this on Bonus level 1 is a good call.

I found the 2nd bonus level to be the easiest on the Easy difficulty setting

This is easiest in the church bonus level on easy difficulty. You will need to hit 100 enemies in a row this might sound hard but it really isn't just takes a bit of practice. Basically if you just pass the first 5 or so kill challenges at the beginning of this it will add up to 100 and you will get the achievement.
