Chests can be anything from the big typical chests to the tiny ones you see round the map. A level is just one segment of the stage not the whole stage itself.
With this achievement be careful not to spray your gun as bullets unlock the chests.
A lot of this achievement is RNG too you want to hope for a small map with not too many chests.
I did this with the Ninja due to the fact he starts with a melee weapon. I went round striking enemies trying to lure them away from any chests and as soon as the portal appeared I went straight inside it. If the achievement popped you've done it and if not you will have to try again.

Chests monsters do not count, so you can kill them without having to worry about it.

You have to not open any chests in an entire level. This means not just walking on top of them but also not shooting them. Chests include the obvious treasure chests with weapons in them but also the orange ammo box and blue experience box too. Just keep trying on level 1-1 and you will eventually get a small map with only a couple of chests. This is shown at 3:49 in the video.
