To clarify from the other two guides, you do not need to do this on the 'EAGLE' challenge, which is able to be played the moment you start Campaign mode.
From the middle hexagon, this challenge can be located in the hexagon located two columns directly to the right of the middle hexagon. To unlock this challenge you must complete one of the challenges of any available adjoining hexagon.
To get a 'Great' rating on the jump, you need to get a score of 95-97. To get a 'Perfect' rating on the landing, you need to get a score of 98-100.
It is currently unknown if you can unlock the achievement with a 'Perfect' jump and a 'Perfect' landing, but I would think it would be able to be unlocked that way.

This is one of the specific challenges in the Campaign. You will only get the achievement when you perform this jump in this challenge.
I hit this in the first ski-jump challenge and was really annoyed that it didn't pop, until I realised that it was in another challenge.

This is one of the challenges you have to complete in campaign mode. To get this you have to get a great take off or a perfect landing. A great take off will be easier because that’s 95-97 score. Just try and keep the ball in the middle when you're jumping, and then when the blue meter starts to fill up, press 
when its almost full for a great jump. Just don’t crash and the achievement is yours.
