Pushing in the RS during a scrum or ruck worked for me. Around 10 times on medium.

There are many ways to get a yellow card (your player gets sent off for 10 minutes). You probably won't get it for your first offence, but one way to do it is: repeatedly doing the same thing to give away a penalty (e.g. collapsing the scrum (press 
during a scrum)).

When there is a ruck or scrum, click the 
in and the scrum will collapse. Your player will receive a yellow card. Keep trying until yellow card is recieved. Early/Late tackles can also be awarded a yellow card.
