The Host piles are the piles of corpses you need to burn in order to stop them to respawn. You know it is destroyed if you can walk on it. They are all the way often under Angels. There is a few you can miss along with one Angel after you beat the King one more time (the fight when he got revived 3 times), go to the right before you go to the door on the left. This is also where you will encounter iChing for the last time.

Some addition/correction to the first comment:
During the last boss fight there are 4 hosts' piles, 3 of them have an angel above them. You can destroy the angels and the host piles below with other guns (it takes a long time but you need it for killing all the angels achievement) but for the pile without an angel which is located directly to your right as you enter the combat zone you will need to use the Hellfire flamethrower.
The first time I got there, I believed that it's glitched, because I didn't use the Hellfire and slashing/shooting with anything else didn't work. After finishing the game I reloaded thr level and used the Hellfire which worked fine.

Going into the final level (The Heart) you should be at 85%. As you're walking to the final boss fight there will be a Hellfire flamethrower on the left tucked away behind some pipes. Grab it then walk up to the burning floor, unequip the flamethrower (down arrow), and shoot the H Cylinders to get across. Grab the flamethrower and walk across to initiate the boss fight and immediately walk around and torch the host piles. As far as I know this is the only way to kill the host piles in the final boss fight. You can't kill the angels in the final fight, they die when the boss is killed so don't waste your flamethrower fuel.

You'll come across the piles by the final chapters of the game and the first ones are located on the level "Walkways of Terror". There are some missable ones such as the ones after you defeat traffiking 3 times. Before proceeding you'll want to take the right and you'll find 4 hosts piles along with an ANGEL.
You can replay the chapters you missed any after you beat the game but in order to unlock this achievement case you forget any pile make sure you play on the difficulty you first started killing them otherwise IT'LL NOT WORK.
My tracker was at 71% when I first finished the game. I tried to get the remaining piles on New Game+ and it didn't work because I first started on normal and New Game+ is set on Hard. Then I tried starting a new game on easy to speed things up, triple checking if I didn't miss any and by the end of the game no achievement. I replayed ALL the chapters where there's piles, being absolutely sure I didn't miss any but again nothing. I tried starting a new game one more time on easy, burning absolutely ALL of them and again nothing.
Then I figured maybe I had to replay the chapters on the same difficulty I first started the game in order to trigger the achievement and that's exactly what did the trick. Once I started playing on normal and burned the missing piles the achievement unlocked.
