Do this in story mode for some reason I couldn’t unlock it in survival or multiplayer. Simply get a map with a hammer and make sure you die to a hammer 5 times in a row. Dying to anything else will reset your progress. If you are towards the end of the map without seeing one you can pause then press the “X” button to restart the map and this will not reset your progress

Yeah same here. Got it on first story level. Restarted until hammer is first then die to it then restart again until hammer is first and die to it. Do it 6 times and it unlocks.
