Mines first appear in the sixth level. They are the small black and gray boxes with a blinking red light. There is one near the beginning of level 6 you can jump on and die 3x, they do continue to reappear through the rest of the levels.

If you somehow manage to get through the game without getting hit by three mines use chapter select to load level 6 that has some right at the beginning. Step on three of them then the trophy will pop.
