Unmissable. Unlocked automatically when completing the Main Story mission An Act of Mercy for the Doctor in Red Forest.
Once you retrieve the Regenerator from Dusty Cable Tunnel and return it to the Doctor, you get this achievement after a cutscene regardless of whether or not you complete the optional secondary objective (which just rewards you with extra coupons).
