Here is a map of the necessary caches, maybe it will help you, but for me cache 43 is not taken, and there is no way to cure it.

I play on Series X, got all achievements except this one, first ran through the guide and took all the caches, but nothing came out, deleted all the voices, reinstalled the game, and ran through the caches again, but only with weapons. And I realized that I don't count 2 caches, these caches just don't have enough loot. One with an AKMS under a tree near the Sawmill, the second at the base where the mercenaries are, a cache with an underbarrel grenade launcher. In the achievement progress, the mark stopped at 88 percent. I also have a bug with the Joker's PDA, it's just not near the skeleton and I can't complete the task.

In the new game I was able to get stash 10. On the 5th try.

Patch 1.0.0.8 fixed the stash under the tree near the Sawmill. The achievement can now be obtained on Xbox.

Version 1.0.0.8 the trophy can be obtained. But there are bugs sometimes not all caches have full loot. In such cases, a new game and immediately to the cache. It was 94% the last cache remained in the forestry in the third installation and there were only cartridges. Created a new game met the first 2 stalkers, etc. to the forestry to pick up the AK and cartridges the trophy fell out in a couple of seconds.

At the moment it is impossible to get the trophy. The cache with the Machine Gun near the Forestry does not count because the cache does not contain 2 underbarrel grenades.

The trophy is cumulative. On a new save I decided to run through all the caches on a beginner, but it dropped out before I ran through all of them because I found some caches on my first playthrough. So if there are missing items in some caches (I couldn't get a grenade with the viper), then create a new game and run to that cache and check.

Just in case, check if a notification popped up after collecting items from the cache: the game played a dirty trick on me and in the cache with the "Viper" a grenade fell under the textures, as a result of which the cache was not counted as collected.
