Counter attacks are performed when the megamonster charges you. Absorbing a monster attack will lead to the monster charge. Which leads to a quicktime event, where you complete hand gestures to strike the enemy. Use nothing but those events to defeat the monster.

A couple things worth noting:
1) Getting hit by one of their attacks seems to trigger the charge; and
2) After the "punch" sequence you CAN do the two sword strikes.
The achievement will pop just before your stats screen.
