This is easily accomplished in the second level of the DLC. Play through the level and if you die, you can easily restart the level by ending the mission and try it again. If you are having trouble with this, try playing on casual, making it easier.

This is during the second story mission ‘Meet the Dominatrix’. This achievement is really easy on casual and with all the major health related upgrades. I even managed to grind out my three weapon kill achievements without getting more than a few scratches and those healed really fast.

This isn't too difficult if you waited until you have all upgrades and weapons, as you'll be extremely difficult to kill. This mission consists of a few sections, with waves of enemies and many mini boss battles with the Dominatrix. As long as you are competent with the game, you shouldn't die and you'll get the trophy upon completing the mission.
