For me this achievement got stuck and yes I did play the mission without codes.

I had to do a new save then work up to that mission. I completed the mission again, this time the achievement popped up.

Story Related and cannot be missed.

This is the mission where you rescue Kinzie from the 50s world, the trophy will unlock afterwards.
