How to unlock the Fourth and Forty achievement in Saints Row IV - Definitive Guide
For this achievement you need to spend 40 hours in the simulation. All you need to do is load up the game, use a plug and play cable with your controller and let it run over night. No need to rubber band the controller. I used this method to get Switch Hitter and Back in the Day also. After finishing all other achievements I still only had 37 hours on the clock. And as others have stated you need to be in the simulation, not on the ship.
Other players have confirmed that multiple playthroughs will count towards the 40 hours. Just needs to be 40 hours in the simulation.

It definitely doesn't always count properly even if you just stand still in the simulation (no pause screen or map or anything, just standing on a roof). First time I had it on for about 8 hours it counted all of it and went from 22 to 30 hours in stats, then the next time it went to 40 hours on the save, but still only 30 in the stats. Then to 47 hours on save file and 37 hours in stats, then 54 hours on save and still only 37 hours on stats..
For me it only works half the time doing the same thing. What's worth noting though, is that when I checked the last time, I was disconnected from xbox live (which might have happened early). So I have a theory that maybe it doesn't count if you get DC'ed from live, but not sure if i was the other times.

just one thing i want to add to the above great solutions is if you press the back button go to extras and then stats at the very top first stat it says total time played. this is how long you have been in the simulation and i can confirm this as i just unlocked this achievement and my total play time reads 40:00:09

You can also just manually save the game and it will tell you how long you have been playing

Nevermind, this is not always true depending on how long you spend on your ship and missions outside the simulation

This achievement is earned when spending 40 hours in the Simulation, and not in the real world (including the ship and real world missions). I thought my game was glitched when I saw that my save stated I spend 32 hours, but in my stats it said 27 hours.

i have 42 hrs but mine hasn't popped yet do i have to beat the game 1st?

Are you sure those are hours spend in the simulation? You can check these in the Stats menu, under Extra when pressing Select. For me it popped exactly at 40 hours simulation gameplay, while my overall time spend (which you can see when you save your game) was 51 hours

If you are leaving the game on and walking away to rack up time in the simulation it can be done with a wireless controller that doesn't have a play and charge cord. What I did to get around the auto shut off was to hold down both RT and LT with rubber bands.
This will cause your character to simultaneously aim and shoot his/her weapon. I did mine by standing away from people and shooting into the ocean with SMGs (infinite ammo).
**If holding both triggers down doesn't work by itself, you can try to also have the controller resting on the camera thumbstick to keep it in rotation.** Thanks to Proscaline for this tip.
Wireless controllers seem to turn off either due to inactivity or lack of change in an input for a prolonged period of time. This is why they will shut off when a thumb stick input is constantly held. Since you are aiming and shooting with this method, I think it satisfies the controllers input change parameters and overrides the automatic shut off.

If you're like me and breezed through everything except challenges within a 24 hour span you still have 16 hours left to go. I am still working on challenges, but that'd still leave me with probably 14 or 13 hours left. The best way to do this is to either just have fun blowing everything up and going crazy, or just leave your system running and make sure your controller doesn't shut off. Not much else can really be done.
