In one of the very first levels in the prologue, if you don’t set a wooden box or other platform down and have her walk off of the rock she is on (press “X” before setting down any boxes) she will fall off of the rock, and apparently, that’s high enough for gravity to rear it’s ugly face!!

This is the most common death and you will likely die this way many times along the way if you don't use a guide. If you do use a guide you may have to force this one. Thankfully you can load almost any level, such as Chapter 1 - Level 1, and just press 
to walk off the platform and die, unlocking the achievement.
