Easy enough with two controllers in a pick-up game. Just pitch with the 2nd controller to the first until they have two strikes on them then bunt with the 1st controller and keep rounding the bases by tapping LT, as the 2nd controller is no-one they'll just stand there with the ball as you get an inside the park home run.
This method can also unlock the inside the park homer achievement as well. Should be simple enough either way. Getting it legit is only a little bit harder than that.
